CASE HISTORY / CLINIC:
Patient with marked xerostomia (dry mouth) and slightly painful,
symmetric, right-sided preauricular swelling.
Discharge of slightly clear secretion from the orifice of the Stensen's duct upon massaging the gland. In addition there's an even more pronounced dryness of the eyes and discomfort in the proximal interphalangeal joints.


FINDINGS:
The parotid gland appears honeycomb-like and hyperdense in the T2 image. The cystic portions appear partially hyperdense or hypodense. The parotid gland appears extremely hypodense on both sides in the non-contrast T2 image.
The parenchyma enhances after contrast administration while the cystic portions remain hypodense.


DIAGNOSIS:
Sjörgren's syndrome


DIFFERENTIAL DIAGNOSIS:
Lymphoepithelial cysts
