CASE HISTORY / CLINIC:
54-year-old female presenting with tachykardia and dyspnea.



FINDINGS:
Suspicion of cardiomegaly based on the fact that the heart-thorax ratio > ½ and the bifurcation angle > 50°.
Bocksbeutelform of the heart with a very mild outline of the heart.
The diaphragm is not well defined on both sides. Large homogenous shadows on both sides which taper off in the supine position.
Prominent apical pulmonary vascular markings depicting cardiopulmonary congestion.


DIAGNOSIS:
Pericardiac effusion, large pleural effusions on both sides and cardiopulmonary congestion.
