CASE
HISTORY:
A
35-year-old patient complains of severe headache after a car accident,feels
better again after an hour. Becomes somnolent two hours later. The clinical
examination reveals a left-sided subcutaneous
parietooccipital skull cap hematoma.
FINDINGS:
Unenhanced
CCT-scan (i.e.
without contrast administration).
Left-sided asymmetric
swelling of the scalp in comparison to the right side. Left-sided hyperdense
(60-80HE) biconvex mass between
brain surface and skull cap. Flattened
gyri und sulci. Midline
shift to the right. The left lateral ventricle is poorly defined.
DIAGNOSIS:
Subcutaneous
swelling of soft tissue with discrete
left-sided parietooccipital hematoma.
Acute left-sided
parietooccipital epidural
hematoma with consecutive mass
effect and midline
shift to the right.
Brain
edema.
Compression
of the left lateral ventricle
