ANAMNESIS
/ SYMPTOMS:
A 75 year old patient complains about an acutely occurred swelling in the area
of the left submandibular space. She shows a mild fever. She reports about pain
while ingesting food and a beginning lockjaw since the day before.
FINDINGS:
The
left submandibular
gland is
clearly hypoechoic changed and alltogether of inhomogenous echogenity with hypoechoic
areas and hyperechoic
shares.
A weak distal echo amplification is discernible. Running along the floor of the
mouth and caudal of the tongue a hypoechoic
band is
visible that reaches the mental
region with
its ventral ending. In the hypoechoic band hyperechoic
reflections with
a slight distal signal enhancement are visible. The hypoechoic band is located
cranial of the mylohyoid
muscle.
DIAGNOSIS:
The patient
has an acute
sialadenitis with
multiple sialoliths as the underlying disease.
DIFFERENTIAL DIAGNOSIS:
Purulent sialadenitis of undetermined origin
