ANAMNESIS
/ SYMPTOMS:
A 52 year old patient shows a big swelling in the area of the right upper jaw.
The swelling continues on the cheek and tongue. The symptoms are increasing for
one week. In spite of antibiosis there is no improvement.
FINDINGS:
In
comparison of both sides on a longitudinal plane a thickened and hypoechoic masseter
muscle of
the right side is visible. Caudal of the masseter muscle the hypoechoic
surface of
the mandible is visualized. The surface shows an interruption on
a distance of about 6 mm. When pressure is exerted with the transducer moving reflections are
visible that disappear into a hypoechoic or echofree space between masseter muscle
and lower jaw bone.
DIAGNOSIS:
The
patient is suspected of suffering from a dentogenous
osteomyelitis with perforation through the mandible. An osseus defect
with abscess formation is visible in the area of the mandibular ramus. Further
orosurgical diagnosis confirmed the tentative diagnosis.
