CASE HISTORY / CLINIC:
Patient with impaired vision and neurological deficit of the abducent nerve as well as headache on the left parietal side that has been increasing in intensity during the last months.


FINDINGS:
High signal intensity, slightly poorly circumscribed petrous apex and clivus in the post-contrast T1-weighted axial image. The portion of the internal carotid artery that lies in the skull base is affected.


DIAGNOSIS:
Meningeoma of the petrous bone that spreads to the clivus.


DIFFERENTIAL DIAGNOSIS:
Gradenigo's syndrome.
Malignant mass lesions.
