CASE
HISTORY:
A 35-year-old patient with severe headache after a car accident, feels better
again after an hour. Becomes somnolent two hours later. The clinical examination
reveals a left-sided subcutaneous
parietooccipital hematoma.
FINDINGS:
Unenhanced
CT-scan (i.e.
without contrast administration).
Left-sided asymmetric
swelling of the scalp in comparison to the right side. Left-sided hyperdense
(60-80HE) biconvex epidural mass. Midline
shift to the left between brain surface and skull cap. Left
ventricle not visualized.
DIAGNOSIS:
Subcutaneous
swelling of soft tissue with discrete
parietooccipital hematoma
on the left.
Acute left-sided
parietooccipital epidural
hematoma. Consecutive mass
effect of the left-sided epidural hematoma with midline
shift to the right.
DIFFERENTIAL
DIAGNOSIS:
Acute/chronic
subdural hematoma
Hygroma
Epidural/Subdural empyema
Meningeoma
